The first several pages are detailed instructions for getting certified copies of your documents. Apparently, in California, you are unable to get a notarised copy of your passport. Just be aware of this. If your original documents are not in English, you must have official English translations.

If your name is spelled differently than your birth certificate, an affidavit is needed.
